Are the trailing arm hex nuts lock-nuts or just regular M14x1.5? The ones I took off my car appear to be lock-nuts but I do not know if they are original.

Ahh. I do have Bruce's awesome rebuilt pedal set. Guess it's time to order the nuts from him as well.

When you guys say no washer. Does that apply to both sides of the pivot shaft? On the diagram it looks like one side only?

The challenge when pulling old stuff apart is that one does not know if it was worked on prior. Thus, there are sometimes conflicts between what you see on the car and what the diagram shows.

IIRC the pivot shaft has serrations on it to prevent it from turning so you wouldn't put a washer there......is that what you were asking? My take on the BMW nuts is that they're deformed to lock them but that may result in screwing up the threads on the pivot shaft during removal---wouldn't use them myself....
