Not enough info yet...

What do the other plugs look like?

Does the fouled plug have a gap? Hard to tell in picture.

But, without any info...

Compression test
Ignition spark intensity test, color of spark across a 1/4" gap
Try another plug
Fuel tests, pressure, air leakage, maybe fuel quality (if old with alcohol in it).
Injector spray pattern tests

I agree with the Hawk, the lean plug would bug me.
How long has it been in?
When is the last time the car ran well?
