Really well done and thought out. Yes - I am fine as "check collector".

I got to thinking after my first post that I could actually host an online form where the party could just enter the data live on the form and click submit - it will then go to a central database - it would remove on step in the process - plus it will be easy to run reports once it is in a database. It will take me a day (noon Friday) to get the online form completed - tested and on line. I will use all the boxes/questions from your form.

By tying their name in the last box - we could accept that as a signature (when worded that way) or add "By Submitting this form you agree to the terms...."

Thoughts?
