Is it possible to change out a distributor on a 914-6 with the engine in the car, or is it a big PITA and/or impossible?

It is possible, and easy. Helps to have a shorty open end 13mm to get at the nut that holds the retaining plate/hold down in place. You have to work by feel, like many things when working on 914s.
