What tranny is this? Does is have LSD? Or ? I have not seen the tranny#'s printed like this before? What model is it? 901? 915?

914/01 is the transmission used in the 914/6. The serial number indicated it is most likely from a 1970 model year 914/6. The serial number location is slightly different from the 914/4 transmissions. This is one of the differences between a 914/6 and 914/4 transmission. The case uses a different casting than is used on the 914/4. You will not find this special serial number area on a 914/4 case.
It is a 901 based design. If it had a LSD from the factory, it should have a "10" stamped between the "914/01" and the serial number. Regardless of how it is currently market (with or without a "10"), the differential could have been converted to LSD or even removed at a later date. In short don't trust the stamping. The only way to 100% know for sure if it has a LSD is to take off the diff cover and look inside.
