Need some help identifying this brand/make of injector. Thanks! Oh and looking for one since I have one that wants to fail when the car gets warm (tested by swapping locations and failing to fire moves).

That looks like a stock 2.0 injector. Possibly rebuilt by Bret Instruments?

PS - You may have to contact them by phone to see if you can get them to rebuild a set of 2.0 green top injectors for your 2.0, & they may need you to send in the cores & wait to rebuild them - since they don't list them as available (only 74-75 1.8 come up on their product search).
