How many small metal inserts (small tubes where the 4 mounting screws go through) are there in the wiper/turn signal switch? I only have 2, should there be 4?

Each screw should have a tube to prevent distorting the switch body when the screws are tightened down. So 4 tubes are needed.

Think of the screws as holding the switch in place but not pulling it down into place.
