Does it matter what side of the cam plug is facing out toward the flywheel?

Smooth side out. Dish faces in.

Doesn't matter.
IIRC standards would face out and automatics would face in (bus T1, T3 and T4 had autos), but that was only because the drive plate could touch the plug.
