What is the Porsche Version print date on the reverse of the cover page?

Does this include the complete unused maintenance log or is it just the cover page as pictured?
If so, is the inside log for a 914-6, and does it have the 914-6 fold-out maintenance schedule pages intact? Would you add a picture of the inside pages please.
