Any idea what would cause it?

seized valve guide?

Piston hit valve? Over-rev valve float?

A valve that didn't move versus a camshaft follower that did.

Either of the above is a possibility. I'd start with a leakdown check on that cylinder.

Very unfortunate discovery. Is it a newly rebuilt engine?

JCS build put in by the PO right before I bought it. Probably 20k miles on it now. Some questionable choices like bus heads.
